Contract and Agreement |
This
contract is being made between FunDiscJockey and

FunDiscJockey agrees to provide recorded music as entertainment
on this date
,
from the time of
until
and at the location of |

| FunDiscJockey will
set up in advance and provide all necessary sound equipment to fulfill
this engagement. If FunDiscJockey experiences equipment break down
and cannot Finnish this show, the fee will be pro-rated determined on
how much time has been played. This will not apply in cases where persons
or events at this show cause damage. FunDiscJockey will not be responsible
for electrical problems or power failures unless directly caused their
actions. |
Customer
must provide access to standard power outlet (With in 25 feet) and one
4 foot table (for equipment.) Customer must provide
for safety of the FunDiscJockey staff, Equipment, and Guest against
bodily injury or damages caused by unruly persons, DRUNK or otherwise,
at this event. Customer shall pay full restitution for damages and/or assist
in the identification and prosecution of person's responsible. |
FunDiscJockey

This
is to assure our protection and proper direction as to who says (turn
it up or down, or play this or that) with out unnecessary hassles from
the crowd. We will do whatever possible to accommodate your guests
and to take their requests. However, we reserve the right to music
selection for the good of the majority and flow of the show. |
Customer
is responsible for crowd control, entry and exits, and providing Liability
and Medical Insurance coverage (most often provided by catering facility)
for the event and its activities related thereto. |
The fee for FunDiscJockey's services shall be $
.00 Overtime
may be obtained at a rate of $150.00 per hour. |
A
down payment of $
is required in advance with
the balance
Final
payment is due at least a week before the event. There will be a fee
of $35.00 for any returned checks. |
REFUNDS
SHALL NOT BE MADE. If cancellations are made for reasons other
than acts of God, death or serious accident, Customer is still responsible
for full contracted fee. Should this event experience a time change
and the event must start earlier than originally projected. The
customer is responsible to pay for any extra time played. Regardless
of what time contracted. |
Should
the event be delayed as a result of FunDiscJockey's late arrival,
double the time delay will be made up at the end of the event or the
equated monetary time difference will be deducted from the fee. Please
sign and return a copy along with the down payment and exact directions
to this event. |
